License: Hearing Aid Dispensers

Major Requirements for License: By Examination -- 1) Completed at least a four-year course in an accredited high school or passed the general education development tests and copy of high school diploma, GED certificate or post-secondary degree; 2) Pass written and practical hearing aid dispensing examinations given by the Department of Health Services; to pass, applicant needs score of 75% or above on both parts of exam; Exam Fee - $250; 3) List of each state that has issued applicant a hearing aid dispenser license; By Reciprocity -- 1) Copy of current license; 2) Name of each state that issued applicant a current hearing aid dispenser license, license number, date license was issued; 3) Statement, on the letterhead of the government agency that issued the hearing aid dispenser license and signed by an officer of the government agency, that the applicant holds a current hearing aid dispenser license in good standing; copy of the state statutes and administrative rules for hearing aid dispensers; copy of the written and practical portions of a hearing aid dispenser examination taken by the applicant or a detailed description of each portion of the examination; state agency's statement of the applicant's score on each section of a hearing aid dispenser examination, minimum passing score for each section, and minimum passing score for whole examination; 4) Application Fee - $100 License Fee - $100; Renewal (Annually) - $100 Continuing Education - Twelve (12) Credit Hours Annually
