If you do not qualify for licensure by Universal Recognition or Credential, use this application to apply for licensure as a psychologist. PLEASE NOTE: The supervised temporary license IS NOT an independent license and DOES NOT LEAD to an independent license in Arizona. The supervised temporary license is issued under A.R.S. § 32-2073. The temporary license does not in any way or at any time transfer to an independent license. If in the future you wish to be licensed to practice independently as a psychologist in Arizona, you will need to apply anew, pay the application fee and meet all requirements then in place. Your training director needs to complete the bottom section to verify that the information you provided is true and correct according to the training records. INTERNSHIP: Supervised Psychology Internship or Training Experience Verification form Needed if you are applying via the Psychologist Application for Exam and/or Licensure form and have been licensed less than 20 years. POSTDOC: Postdoctoral Professional Psychology Experience Verification Needed only if you are applying postdoctoral hours towards licensure. If you have been licensed in another state for 10 years or more, you do not need this form, because you may waive the requirement to have additional hours verified. The universal recognition policy (A.R.S. § 32-4302) allows Arizona residents to use an out-of-state professional or occupational license to qualify for an Arizona license to work. To qualify, an applicant must:Prove residence in Arizona (See the Board's Substantive Policy Statement regarding this). You MUST establish residence in Arizona and be able to provide evidence of this. If you have not established residence in Arizona, or you answer NO to the statement, "I am a resident of Arizona", you DO NOT QUALIFY to apply for licensure using this application. If you have a recognized credential (ABPP, CPQ or National Register) and have been licensed for 5 or more years in another state, use the Credential application. Otherwise, use the standard app, Psychologist Application for Exam and/or Licensure. Be currently licensed or certified for at least one year in another U.S. state or territory in the discipline applied for and at the same level of practice as recognized in Arizona. Be in good standing in all states where currently or previously licensed or certified. Verification is required directly from the licensing board to the AZ Board of Psychologist Examiners for all licenses or certifications ever held regardless of the status of the license or certification. If you are applying for a fee waiver, do not include the application fee with your application. Board staff cannot accept the application fee with the application for fee waiver. Under these circumstances, any checks or money orders will be returned. Board staff will email you regarding your fee waiver application once review is completed. To pay the application fee by:Credit or Debit Card: If paying by credit or debit card, please submit the application via the psychologist submissions portal. After your application has been entered into the Board's database, instructions will be sent by email. Due to the backlog we are currently experiencing, please allow 30 days for processing. Check or Money Order: If paying the application fee by check or money order, please mail your application with your payment to the Arizona Board of Psychologist Examiners. (Please make the check or money order out to the AZ Board of Psychologist Examiners.) It is recommended that a delivery service is used (i.e. FedEx, UPS, DHL, etc.) when sending checks or money orders. The Board is not responsible for lost mail. TOP What You Need to Know About the Application Process:A doctoral degree in an applied psychology is required for licensure as an independent psychologist in Arizona. Please see the detailed education and training requirements in A.R.S. § 32-2071. Board staff strives to provide helpful information, but due to the backlog in applications at this time, is unable to answer individual questions that are already answered on the Board's website. Please see the Table of Contents at the top of this page to review resources provided by the Board relative to requirements for approval to sit for the EPPP and/or licensure. All applicants using the Exam and/or Licensure application must meet ALL the requirements of A.R.S. § 32-2071. There is only ONE exception to these requirements. A temporary waiver of doctoral program residency requirements as a result of SB1253 was passed by the Arizona State Legislature and approved by the Arizona Governor on April 9, 2021. This new, temporary law allows that from January 1, 2021 to September 1, 2023, the residency requirements in subsection A(5) and (K) of A.R.S. § 32-2071 are waived. The text of this waiver may be found in SB1253, on page 30, line 16 (Sec. 15. Licensure requirements: exemption; retroactivity). It is the applicant's responsibility to assess that their doctoral program and supervised experiences meet Arizona's requirements. Board staff cannot review your transcript and advise regarding this. Please see the FAQ regarding doctoral programs to assist you in making this assessment. Don't assume that the Board will be able to make an exception for you regarding a statute or rule requirement. Licensing Boards can do only that which is authorized in law. If the exception is not prescribed in law, the Board cannot do it. Please make this assessment before sending an application in and paying the application fee. We do not have online initial applications at this time. All applications and forms on the Board's website are created in Adobe Acrobat and are form-fillable. This means the applications have form fields for typing your answers. Applicants and verifiers should complete them in Adobe Acrobat, Adobe Reader, or other Adobe PDF app/program to see all the fields and used the digital signature blocks. Yes, digital signatures are acceptable. DO NOT open and try to fill out any of these applications or forms in an app or program that alters the formatting! All other programs and apps change the functions of the document. Applications or forms that have been altered from the original formatting will be rejected, delaying the processing of your application. You may print the application and complete it "by hand" as long as your printing or handwriting is completely legible. Your g's and y's, 2's and 5's, 8's and 3's, etc. must be easily distinguishable. If the application can't be read, Board staff will require a re-do, further delaying the processing of your application. PLEASE READ EVERYTHING. Do NOT just scan and assume. Applicants for licensure are responsible for reviewing the statutes and rules that prescribe the various application options and for understanding the requirements of that for which they are applying. These sources of authority are available on the Board's website. It is the applicant's responsibility to complete the application materials with accuracy and thoroughness. Use the first page of the application packet as your checklist relative to the supporting documentation needed to administratively complete your application. Please note, if you answer "yes" to any professional conduct question, appropriate documentation is required. Please see the PDF, "Appropriate Documentation for 'Yes' Answers" for a list of documents the Board requires for each professional conduct question that may be found in the psychologist applications. Your application cannot be considered administratively complete without the appropriate documentation. Board staff cannot advise what type of application for you to make, what answers you should give, or provide an interpretation of the statutes or rules. If you need advice, please contact a colleague, mentor, supervisor, or other experienced psychologist for this type of assistance. You may also wish to contact an Arizona association dedicated to supporting and advancing the psychologist profession in Arizona. Links to several associations have been provided on the Related Links page of the Board's website. Consult an attorney for an interpetation of the statutes and rules. Before submitting your application, save a copy for your own records. Please upload the application to the secure submissions portal or mail it using a delivery service. Security cannot be guaranteed with email, and regular mail cannot be tracked. The Board is not responsible for lost mail. See How to Pay Application Fees for fee payment information. To see and use the digital signature block, open the application in Acrobat or Reader. Time frame from receipt of application to approval averages 5-6 months and is entirely dependent on how quickly verifications are received. For example, if you wish to be licensed in July, please send your application in January. Applicants may request verifications or other documentation sent to the Board in advance of their application. Verifications, transcripts, and any other documents received in the Board's office (by submissions portal, email, mail, fax or delivery service) are retained for 12 months and are matched to the application once the application is received. Verifications for supervised experience hours must be uploaded by the verifier (or verifier's staff) to the the psychologist submissions portal. If a firewall prevents use of the portal, please have the verifier email the verification directly to [email protected]. License verifications and official transcripts may be emailed to [email protected]. Due to the state of emergency, please allow up to six (6) weeks for staff to complete the initial intake of your application and email you a a notice of incomplete application (or administratively complete application if it is complete). A.A.C. R4-26-201 requires that an application be placed on an agenda if it is administratively complete 18 days prior to the next scheduled board meeting. This also means that an application that is not administratively complete cannot be placed on an agenda. "Administrative completeness" means that all required verifications and documentation have been received and processed by Board staff and the application fee has been paid. Please be aware that staff has up to 100 applications open in various stages of completeness at any given time, and applications and documentation are processed in the order they are received. If any item has not been received, you will receive a notice of incomplete application and your application will be placed on the next available agenda AFTER the missing documentation is received. It is the responsibility of the applicant to request an update after the initial notice of deficiency has been emailed to the applicant by Board staff.
