License: PT Temporary Permit

In order to obtain a temporary permit as a physical therapist, an applicant must provide the following documentation: A completed application with photograph and fee of $285.00. Applications are only accepted online. Please select this link to apply onlineOpens in a new windowOpens in a new window; A completed temporary permit application form; A transcript verifying the award of a degree in physical therapy and date of graduation sent directly from the college granting the physical therapy degree. Please note that for purposes of issuance of a temporary permit this office will accept a letter from an appropriate official (such as a Registrar, Dean or Program Director), on official university letterhead, specifying that all academic requirements for graduation have been completed and the date on which the degree will be conferred. A final, official transcript verifying completion of the degree program is required before a license will be issued. Foreign trained applicants are required to arrange for the submission of a credentials evaluation sent directly to this office by the Foreign Credentialing Commission on Physical Therapy (FCCPT) prior to being eligible for a temporary permit.
