To receive a Dentist License in the State of Michigan, an applicant must graduate from a CODA/ADA-accredited dental school, pass Parts 1 and 2 of the National Board Dental Examinations, and pass the ADEX Dental Examination (including the periodontal section). After meeting these requirements, the applicant must submit a Dentist License Application along with the appropriate fee ($127.25).
